Protecting fetal development – project evaluates a new early pregnancy blood test

28.10.2025 - A joint pilot project is being launched in the wellbeing services counties of Pirkanmaa, Kanta-Hämee and Etelä-Pohjanmaa. The goal is to support the well-being of pregnant women and ensure that the fetus develops in a safe and substance-free environment. The project will assess whether a new, more sensitive PEth blood test is suitable as a routine screening method for alcohol use in early pregnancy.


The sensitive PEth blood test (B-hsPEth) can detect even occasional alcohol use from about 2–4 weeks before the sample is taken. It is more reliable than self-assessment methods for alcohol use during pregnancy and measures lower blood PEth concentrations more accurately than the standard PEth test currently used in healthcare. The standard PEth test is mainly used to detect long-term, above moderate alcohol consumption in non-pregnant individuals.


Blood samples for the sensitive PEth test will be taken together with other early pregnancy blood tests, usually during weeks 9–12 of pregnancy. Information about the sampling will be provided during the first maternity clinic visit. The collection of samples will begin at the start of November.


The results of the sensitive PEth test will be handled completely confidentially as part of routine maternity care. If the result indicates alcohol use, a new blood test will be taken three weeks later. If the second result is also positive, a more detailed assessment will be made, and the pregnant woman will be offered support to maintain sobriety according to the region’s care pathways.


The pilot project is carried out as part of research supporting the decision-making of the Finnish Government. The results will be used to develop a national model for early pregnancy alcohol screening.
